Certificate of Parents' Income Condition:<\/p>\n\n\n\n<p>a. For parents\/guardians who work in private institutions\/companies must submit: &nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Evidence of a decrease in turnover from the head of the institution\/company where you work, or<\/li><li>Proof of income of parents\/guardians before and after the covid-19 pandemic from institutions\/companies, or<\/li><li>Proof of Termination of Employment for parents\/guardians who work in private institutions\/companies.<\/li><\/ul>\n\n\n\n<p>b. For parents\/guardians who work or have a business\/self-employment from the informal sector that has receded due to the pandemic, make a statement authorized by the Head of Village\/Lurah.<\/p>\n\n\n\n<p>3. Copy\/scan of Family Card.<\/p>\n\n\n\n<p>Application letters and supporting documents are uploaded no later than August 12, 2021 through&nbsp;<a href=\"https:\/\/isi.ac.id\/bantuan-ukt\">https:\/\/isi.ac.id\/bantuan-ukt<\/a>&nbsp;The submission letter will be verified by the Verifier Team.
